Professional Case Management
Your care requires continual communication and review. Metron Post-Acute Rehabilitation Program
assigns a registered Nurse Case Manager to coordinate the efforts of our skilled healthcare
professionals in your treatment. Weekly meetings of the entire case team will be conducted to
review your progress in the direction of your established clinical goals, in addition to
coordinating your future care needs. A single contact person will also be provided to respond
to any of your questions.
Communication
Because excellent care does not stop with treatment, our Registered Nurse Case Manager will
communicate the status of your care with the referring physician on a continual basis. Through
constant communication, Metron Post-Acute Rehabilitation Program can ensure you get the most
effective care possible.
